It's an unfortunate tragedy, so sympathies to all who were affected.

I'm not particularly interested in aviation, but it's eye opening to see how such incidents happen by the unfortunate accumulation of different mistakes: how the holes in the Swiss cheese ligned up!

We live in and operate complex systems, and sometimes I ask myself how much are we able keep the whole picture in sight.
